Telehealth as a Vehicle, Not a Destination

Dr. Sandrow’s case for telehealth isn’t that it’s the perfect career; it’s that it solves the most immediate problem physicians face when leaving traditional practice: income replacement. She doubled her income and cut her hours in half within two years of going full-time. The flexibility that came with it gave her space to figure out what she actually wanted her life to look like, something burnout rarely allows.

The business ownership piece, however, is something most physicians have never been trained for, and Dr. Sandrow treats it as non-negotiable. Taking a straight employment position with a telehealth company solves the income problem but trades one set of constraints for another. Building your own structure from the start is what creates actual freedom.

What the Transition Actually Looks Like

Multi-state licensing is the longest lead item and gets started first. From there, she recommends setting up an LLC, preparing for tax implications, reviewing telehealth company contracts with a focus on non-compete clauses, and selecting the right companies to apply to. To optimize income, Cherisa recommends that physicians stack multiple telehealth positions to maximize income and flexibility.

Non-competes in telehealth carry specific risks that differ from traditional practice. A broad non-compete with one company can block a physician from working with any other telehealth platform nationwide, including after leaving that company. Dr. Sandrow addresses this directly and recommends physicians evaluate contracts before signing.

Summary

Telehealth is one of the most accessible and immediate paths available to physicians seeking income replacement and schedule flexibility. Dr. Sandrow built and tested the roadmap herself before teaching it, and the coaching she offers covers every step from licensing to launch.
